Logging in to DataHub FAQs

  • What should I do if I don't receive the "Reset the Password" email?
    First, check your Spam or Junk folder for the email. If it's still not there after a few minutes, ensure you've entered the correct email address linked to your account. If nothing still shows, send us a report so we can look into it.


  • What is the correct website address to log in to the DataHub portal?
    The correct login page is chambercare.datahub.insure/login


  • Why am I being asked for a 6-digit verification code after I enter my email and new password?
    DataHub uses a secure login process that includes two-factor authentication (2FA). This temporary verification code is an added security measure and is sent to your registered email address every time you log in the first time, and if you log in using a new device.


  • I just set my new password, but when I log in, it just sends me back to the login page. Did the reset fail?
    No, the reset was successful. You will be rerouted back to the login page where you must input your email and the new password to begin the login process that will then prompt the 6-digit verification code.



PHQ FAQs


  • When can I send the MHQ form emails to a member?
    Emails can only be sent when a member’s PHQ status is "Pending."


  • If a member doesn't receive the PHQ email, is there an alternative way for them to access the form?
    Yes. You can copy the Health Application link from the group record and send it directly to members or brokers. They will be directed to the PHQ registration/login page.


  • What is the first step a member should take when they click on the shared PHQ link for the first time?
    They should click "Sign Up" to create an account and proceed with the application.


  • How can I track which members have completed the PHQ?

DataHub tracks submission status on your Census Info page. Member statuses will update from Pending to In Progress and finally to Completed as members fill out and submit their forms.
